Navigating the 2026 Medicare Landscape

Navigating Medicare in 2026 brings significant changes and important decisions for beneficiaries, especially in Central New York and nearby regions.

Medicare’s evolving landscape is shaped by several factors. Insurance companies are dealing with stagnant hospital funding, increased utilization after the pandemic, and lower reimbursement rates. Prescription drug costs are rising, especially for popular medications like GLP1s. New requirements for RX caps and payment programs add complexity, and these changes vary by county.

What’s New for 2026?

Medicare coverage details are changing. For Part A (Hospital), the monthly premium remains at $0, but the deductible per event rises to $1,716. Daily copays for extended stays and skilled nursing facility coverage are outlined, with no maximum out-of-pocket protection. Part B (Medical) premiums increase to $206.50 per month, with a $288 annual deductible and 20% coinsurance, leaving beneficiaries responsible for a significant portion of outpatient costs.

Income matters more than ever:

  • Higher earners pay more for Part B and Part D through IRMAA (Income Related Monthly Adjustment Amount).
  • For example, individuals earning over $500,000 pay nearly $500 per month for Part B.
Programs That Help

Several programs support those with limited income:

  • Qualified Medicare Beneficiary (QMB): Covers premiums, deductibles, and copays; automatically qualifies for Extra Help with Part D.
  • Medicare Savings Program (MSP): Approval also qualifies for Extra Help.
  • LIS/Extra Help: Caps copays at $4.90 for generics and $12.15 for brand-name drugs.
  • EPIC (NY State): For residents 65 and older, with income limits, offering reduced RX copays and premium assistance.

Original Medicare leaves gaps in coverage. There’s no prescription drug coverage unless you buy a standalone Part D plan, no maximum out-of-pocket protection, and no coverage outside the U.S. Most beneficiaries add coverage with either Medigap (Supplement) or Medicare Advantage.

Medigap & Standalone RX

Medigap plans G and N cover most deductibles and coinsurance, but premiums are rising. Standalone RX plans also see increases, with the maximum deductible climbing to $615 and more tiers subject to the deductible.

Medicare Advantage

Medicare Advantage plans bundle hospital, medical, and drug coverage, often with lower premiums and extra perks like gym memberships, dental, vision, and hearing benefits. However, changes to ancillary benefits mean some extras now require riders or are less available.

Changes to Part D

A major shift in 2026 is the increase in the Part D maximum out-of-pocket to $2,100, and the introduction of negotiated drug prices for the first ten medications, resulting in significant discounts. The Medicare Prescription Payment Program allows beneficiaries to spread costs over the year with predictable monthly payments, easing the financial burden.

Other notable changes include:

  • Maximum out-of-pocket increases to $2,100.
  • Deductibles rise to $615 and apply to more tiers.
  • Shift from copays to coinsurance.
  • First 10 drugs with negotiated prices (e.g., Januvia, Farxiga, Eliquis) see significant discounts.

Value-Based Insurance Design (VBID), which allowed Medicare Advantage plans to offer extra benefits for healthier outcomes, will expire at the end of 2025. Special Supplemental Benefits for the Chronically Ill (SSBCI) remain, but only for those meeting strict clinical criteria.

Sunsetting Plans & Regional Changes

Many plans in the Syracuse area and Central NY are ending (“sunsetting”) at the close of the year. If you don’t choose a new plan, you’ll have only Original Medicare starting January 1, 2026. New plan options are emerging, and some existing plans are being renamed.
